They created a restroom and hired an actress with her twin sister to act as if they were in the mirror, and in the mirror place they've placed a see trough mirror, so any other person in the room are not able to see their reflection.
The face expressions are the most comic ones, and also the people in the gag seemed to be very shocked and afraid of the situation. Who wouldn't??
